“Dr Pratt has an estranged relationship with his father,” Mekhi Phifer says. “Ray Liotta’s character comes in, a guy who is drinking himself to death, and Pratt at first looks at him with disdain. But as the episode goes on, a lot of things we gradually learn about [this patient’s] past are things that resonate with Pratt about his own life.” “As we delve more into Pratt’s life, and who he is, you’ll see some changes. That’s all part of all the transitions we have going on with the show as a whole, with some cast members leaving us. You’ll see a lot more about Pratt.”

Ray Liotta makes a guest appearance as a rock-bottom alcoholic in the final stages of liver failure, whose final hour is played out in real-time. But as the County staffers work to delay his inevitable fate, they must also contend with a waiting-room full of patients and Pratt’s uncharacteristic aversion to treating what looks to be a lost cause.

In a ground-breaking episode which traces the real-time events in the emergency room, Kovac, Abby and Sam try to save the life of Charlie Metcalf (guest star Ray Liotta), a man who comes in complaining of a stomach ache, but is later diagnosed with far, far worse. Mekhi Phifer and Shane West also star.

ER will take a page from 24’s rulebook this November when it features an entire episode in real-time. The installment, slated to air Nov. 11, revolves around an ex-con (played by special guest-star Ray Liotta) who walks into the ER suffering from withdrawal and hallucinations. “The episode is 44 continuous minutes of this man’s treatment and his interactions with the staff,” executive producer David Zabel tells The Hollywood Reporter. ER first played around with the real-time format back in 1997 when it aired its season premiere live.

A woman is outradged because patients that came in after her are treated before she is. Kovac, Abby, and Sam treat an ex-convinct with belly pain. When his state gets more complitcated and critical, Sam contacts the man’s son; the two of them have not seen each other in years. A kid gets injured because a sibling dropped the tv on him. Ray and Neela take care of a man with lung problems. Pratt and Dubenko also appear.
